What is the requirement for immunizations for a child entering school?

The district needs to know the month, day, and year for each immunization. If you cannot find the day, use the first of that month. However, using the first of that month may put your child out of compliance in some cases where the shot was given close to 12 months of age (MMR) or close to his/her 4th birthday (DPT and polio).
Immunizations must be up to date prior to the first day of school. Washington State Law requires all noncompliant children not be allowed to enter school.
If you prefer that your child not receive one or more of the required vaccines you may sign a personal or religious exemption. A physician's signature is required for a medical exemption.
Students who are susceptible, i.e. not fully immunized, may be excluded from school, by order of the Seattle-King County Health Department, in the event of an outbreak of the disease(s) for which they have not been adequately immunized.
