    2021-22 Facility Use info:

    Individuals and/or User Groups are required to complete and submit a Facility Use application through our on-line facility use system, every time and provide a Certificate of Liability Insurance with Endorsement and supporting documents, a minimum of 10 business days in advance from the first date of requested use to allow for verification, processing and approval by the Operations Department. 

    The new PAC Auditorium is not available for rent till further notice 21-22Yr

    *RENTERS* You can start rental request End of September for Fall Requests!

    *SUMMER RENTALS* NO INSIDE BUILDING RENTALS IN SUMMER! Outisde Field use, Parking Lots are mostly open for applications only!!

    *PLEASE REMEMBER*Field use,Parking Lot Renters are responsible for having Porta Pottys with washing station onsite during usePlease have them dropped off week of Event, and picked up after Event!!!

     

     For Community Use Rental Requests please review the list below of the required items!  

    It is our policy not to hold or reserve facilities until a completed application and all documents have been received.                                                                                                                                  

      Required Items to upload before even looking at for approval                                                                                                                                                              

    1. The on-line request in Community Use Database/Application-filled out completely and attached                                                                                                    
    2. Form - Concussion/Heady Injury/Cardiac Arrest Forms Uploaded to your on-line request (if applicable)                                                                                                    
    3. Certificate of Insurance (with SVSD info as the Certificate Holder section on Form)
    4. Copy of 501c3, NON Profit from IRS* (if applicable)
    5. Portable Restroom Form-(Field use or parking lots)
    6. Food Handlers permit (if applicable)
    7. IMPORTANT*Processing Fee - ($15 must be paid through the on-line payment system* At time of application being filled out and attached, if needed mail, or deliver to the District Office)
    8. Remember***Please have in 10 days prior to Event, and Any missing documents can delay or cause your event to be declined.****

    • Processing Fee $15 (Needs to be paid at time of Application request with receipt attached). Rental Fees can be paid at the following link:  https://www.svsd410.org/Page/4124 (All instruction are included).

     

    • Question on how to attach documents review the CommunityUse Requester Guide. 
    • User Groups involving any athletic activity are required to complete a Compliance Statement form that combines: HB1824, in accordance with House Bill 1824, the Zachery Lystedt law and SB5803 Sudden Cardiac Awareness law. 
    • Room or field fees, energy fees, custodial overtime, security, set up, kitchen use fees or security deposits may apply. Rental charges are billed monthly but payment for applicable charges may be requested to be paid in advance of the event.
