Home Hospital Services
Home Hospital services are in-person tutoring sessions for students who cannot attend school for a period of 4-18 weeks due to sickness or injury. The purpose of these services is to keep students current in their regular academic classes. Services are provided at a student’s home or hospital room at the expense of SVSD.
Any public school students are eligible for home hospital services as long as a request is submitted with the following information:

Student’s Diagnosis
-
Signature of Qualified Medical Practitioner with date and anticipated number of weeks of absence

Requested start date of services
-
Whether or not the 504 or IEP team needs to meet (if applicable)
Qualified medical practitioners are identified as:

Doctor of Medicine (M.D.)

Doctor of Osteopathy (D.O.)
-
Doctor of Naturopathy (N.D.)

Doctor of Dentistry (D.M.D.)

Doctor of Chiropractic (D.C.)
-
Physician Assistant (P.A.)
-
Advanced Registered Nurse Practitioner (A.R.N.P.)

Certified Nurse Midwife (C.N.M.)
