A leave of absence (LOA) is required any time you miss more than 10 days of work.
Whether you are experiencing a family or medical emergency, adding a new family member, scheduling a planned surgery, or experiencing another reason that has led you to take an LOA, we are here to discuss your options.
How to take a Leave of Absence
- Submit a Leave of Absence Form

- Submit Required Supporting Documents
- If Leave Dates Change - Submit a New Leave of Absence Form
- Submit Required Supporting Documents for Return to Work
