Notice to parents of incoming 7th graders for the 21-22 school year Regarding Tdap Booster

The Tdap is a booster vaccine which continues to protect your child against the tetanus, diphtheria, and pertussis diseases they were immunized against as young children. In Washington State, a Tdap vaccine is required for students attending school in 7th through 12th grades. Documentation of the Tdap vaccine or an exemption is required and must be turned before your child can start 7th grade on August 31st, 2021.  Valid documentation includes medical records showing vaccination or a completed Certificate of Exemption (COE) form.

If we do not receive documentation by due date above, by law, we must send you a Notice of Exclusion for Immunization Noncompliance. This will explain that your child cannot attend school until you provide the required immunization documentation, RCW 28A.210.120.

What do vaccines cost?

In Washington, all children under age 19 can get free vaccines from their health care provider. Providers may charge an office visit fee and a fee to give the vaccine, called an administration fee. If you cannot pay the administration fee, you can ask your health care provider to waive the cost.

Where do I turn in the paper copy?

  • You can turn it in to the office when the office opens up to the public August 23rd
  • You can fax it to the building – CKMS fax number for CKMS 425-831-8290
  • You can mail it: CKMS, PO Box 639, Fall City WA 98024 – attention SCHOOL NURSE
  • You can have your student bring the first day of school to the office – we will have a collection site for the paperwork
