COVID Vaccine Clinics for ages 5-11 Available -- Register Now for Nov. 9-20

The CDC recommends that children and adolescents age 5 and older get a COVID-19 vaccine as the best form of protection against the illness. The Pfizer COVID-19 vaccine is now emergency authorized for children and adolescents age 5 and up, as a 2-dose series taken 3 weeks apart. The dose for children ages 5-11 is one-third the dosage of the vaccine for older adolescents and adults.


Our school district is offering free, optional, Pfizer COVID vaccines for our students ages 5-11 years, through a partnership with Snoqualmie Valley Hospital. 


These vaccine clinics will take place at Mount Si High School’s Freshman Campus Commons, at 8651 Meadowbrook Way SE in Snoqualmie, Building 4000, near the “Red lot” (not the “Gray lot” main entrance). See campus map. Follow the stairs to the Freshman Campus entrance. If you need elevator access, please make prior arrangements by contacting with your scheduled appointment time.


Sign up today for first dose appointments:

  • November 9, Tues., 5-8 p.m. (3 weeks later - Nov. 30)
  • November 13, Sat., 8 a.m.-1 p.m. (3 weeks later - Dec. 4) 
  • November 17, Wed., 5-8 p.m. (3 weeks later - Dec. 8)
  • November 20, Sat., 8 a.m.-1 p.m. (3 weeks later - Dec. 11)


These clinics will provide Pfizer COVID vaccine doses for ages 5-11 only.  Read the Pfizer Fact Sheet for more details.


Parental/guardian consent is required and guardians must accompany their children to their appointment. The hospital requires a copy of the guardian’s photo ID, with a signed consent form with screening questions. You can upload these through the registration link below or print copies of the 2-page pdf here -- Consent form (in English) or Consent form (en Español) -- to bring to your appointment completed.

This registration is only for first dose appointments (the first four clinics Nov. 9-20). Second dose appointments will be scheduled during the school clinic after a first dose is administered. After the November 20 clinic, we’ll assess if any appointments remain, and if applicable, reopen the registration for parents to sign up for additional doses (first or second) as needed.


If you are unable to secure the timeslot you want, you will also be able to schedule an appointment to get vaccinated at Snoqualmie Valley Hospital or use the DOH Vaccine Locator/Youth to find locations with appointments. Quantities may be limited at first, until health providers receive their dose allotments, so please be patient.
