School Nurses Need Your Medication Forms, Immunization Updates, Health Care Plans
Although most students are learning remotely, our school district nurses want to be ready for when we can bring more students back in person. We ask that you to complete the following medical forms if they apply for your student, as soon as you are able to do so:
Any new immunizations? – Please email your school nurse information about any new immunizations your child has received (name, type, date received), so that your student's file may be updated. (See 2020-21 Immunization Requirements)
Health Care Plans – If you received a Health Care Plan last spring for your student’s unique health needs, please sign that, update it if needed, and return it to your school nurse. If your student is new to the district and needs a plan created for him/her, please contact your school nurse.
Medication order form – This must be signed by your doctor for all medication(s) needed at school. (Until your student returns on campus in person, please keep the new medication at home.)
These complete forms are needed, so they can ensure your student’s health records are up-to-date and in compliance when he/she returns to school in person.
Return the forms to your school nurse, via:
postal mail to your school’s address,
scan and email your nurse,
or, fax your school. (Check your school website, secretary or nurse for the fax number.)
