Transitioning to New SVSD Attestation QR Code and Link Starting March 22


One important component of our district’s Return-to-School Safety Protocols has involved completion of a daily health screen and attestation in the form of a link sent in a daily email to the Family 1/Guardian 1 email address designated for each student in Family Access. This system also allows parents and guardians to opt into receiving a daily text message, if they prefer, containing the daily attestation questionnaire link that is unique to each student each day they are scheduled to attend school in person. 


Up to this point, our district has been using a shared version of the attestation tool with 65 other school districts in the state. Beginning Monday, March 22, we will be transitioning to a Snoqualmie Valley School District stand-alone version of the attestation system.  This change applies to all students and staff at school.


When the district moves to the new stand-alone version, starting Monday, March 22, the email subject will show: “Action Required: School Daily Symptom Check for [STUDENT’S NAME].”  The attestation emails will originate from the sender:


To prepare for this transition:  Please add this email address to the approved sender list in your email settings now, so the email attestation can be delivered to your Inbox on Monday (and avoid it being sent to your Spam or Junk email filters).


If you have already opted into the text message feature associated with the current attestation process, that will also be an option beginning March 22, but it will be sent from a new number. You should continue to receive text messages with the new link. 


Additionally, if you do not receive the email and/or text on a day your student is scheduled for in-person school, and need another way to complete the attestation requirement for a given day, the QR code and/or link to take the survey will also change.  Starting Monday, March 22, there will be just ONE QR code and link for the entire district, rather than different codes for every building.


The new SVSD Attestation QR code and link – to use as a back-up option to the daily emails and text messages – are posted below.  They will also be posted prominently on the district and school websites, when the SVSD system is activated on Monday, March 22.  Please be sure use the same Family 1 / Guardian 1 email and the student’s name as it appears in Skyward if using the generic link, so the system can accurately match the response with the student’s profile.
