Collecting COVID Vaccine Verification for SVSD Students/Staff
Dear Families of students aged 12 and over,
We hope you have had a wonderful, relaxing summer! We have been hard at work preparing for a strong start to the 2021-22 school year, and we are excited to welcome you back to what we know will be an outstanding year.
According to the latest guidance for schools from the Washington Department of Health (DOH), fully vaccinated individuals do not need to quarantine if they are exposed to COVID and have no symptoms. We are inviting you to share your child’s COVID vaccination information with us so that, in the event of a close contact with a COVID case, your fully vaccinated student can remain at school for learning. For individuals who are close contacts and not fully vaccinated, a 10-day exclusion/quarantine is required.
To help us gather this information, please complete the RSVP Process in Family Access, which includes a Vaccine Verification step for grades 6-12. If your student has not received the COVID vaccine or you don’t want to share this information, you’ll be able to answer “no” or “I prefer not to share.”
For those who are vaccinated:
Complete the Vaccination Verification form found in the RSVP Process in Family Access and upload into the form a copy of the COVID vaccination documentation. A copy of the vaccination card, health provider documentation, or a printout from the state vaccination system is acceptable.
OR, email a copy of your child’s vaccination documentation to the appropriate school email address (listed below), or visit your school office to present the vaccination card in person between August 20-27.

For anyone age 12 or older who still needs a COVID vaccination, you can sign up for a SVSD Vaccine Clinic offering Pfizer vaccinations to all, free of charge, as well as other free school immunizations for students age 11+.
